Integrated Personal Support
As a Connexions Partner, our Personal Advisers are working with individual young people in schools, colleges, communities and Connexions Centres. providing one to one support to keep them engaged in learning, overcome barriers to achievement and help them enjoy what life has to offer.
Personal Advisers help young people to
- Assess their individual needs
- Develop plans to address those needs
- Coordinate specialist support and services
- Address issues and concerns
- Make successful transitions
Working collaboratively with other agencies, Personal Advisers are developing their role within the Common Assessment Framework to ensure the early identification of young people’s needs and by supporting young people to achieve their potential in all 5 Every Child Matters outcomes.
Easykit Assessment Toolkits
Easykit toolkits have been developed through Connexions South Yorkshire to facilitate young people centred APIR and Common Assessments (CAF).
Easykit products and services are used by a wide range of professionals working with children & young people, to conduct effective, comprehensive assessment of need and to measure the impact of work undertaken.
